2. What is this medication for? Does this take the place of anything else that’s currently being taken?
3. Are there any duplicate or unneeded medications?
4. How and when should this medication be taken?
5. What should we do if we miss a dose? What if we accidentally take too much?
7. Should the patient avoid any other medicines, dietary supplements, foods, or activities while taking this medication?
8. How should this medication be stored?
9. How long do these medications need to be taken?
All this information should be on the accompanying leaflet so take time to read it, and write down any specific pharmacist recommendations (e.g., take with/without food) that aren’t.

I agree it is often on the medication pamphlet and/or pharmacist print out. Albeit, more pharmacist are going paperless (yikes!). I have also learned and witness many individuals do not read these very important documents, and rely solely on what their provider says, and what is written on the prescription bottle.

All too true. These are great questions; I wish more providers would also note on the Rx that there should be a pharmacist consult.
